Diagnostic endoscopy in the patient with chest pain

Abstract

In an investigation into the incidence of gastrointestinal pathology in patients referred to hospital with a uspected diagnosis of ischaemic heart disease, 83 patients were selected from 700 for endoscopy on the basis of clinical uncertainty as to the origin of their symptoms. Twenty-two out of 83 were shown to have gastrointestinal disease alone; 41 out of 83 had gastrointestinal pathology in addition to ischaemic heart disease. All 22 subjects with gastrointestinal disease had positive endoscopy findings, 17 of whom obtained complete relief of symptoms with cimetidine or antacid therapy.

It is suggested that fibreoptic examination of the upper gastrointestinal tract is included earlier and more frequently in the investigation of patients presenting as cardiac cases where additional supportive evidence of heart disease is not convincing.
