Ventriculo-atrial conduction over accessory pathways exhibiting decremental properties

Abstract
Recent evidence has suggested that some anomalous ventriculo-atrial (VA) connections may exhibit decremental conduction. These pathways may provide an anatomical substrate for tachycardia, but may also be discovered incidentally at study. In this report we describe two patients in whom electrophysiological studies suggested the presence of an atypical VA pathway. In one case ventricular extrastimulation induced eccentric atrial activation with progressive VA delay. In another patient, these features were seen in addition to a sudden jump in VA conduction time suggesting the presence of an anomalous pathway exhibiting functional duality. In neither case could tachycardia be induced.
The implications of these findings are discussed and the literature reviewed.
