Intracardiac electrophysiological study of disopyramide in intact and chemically sympathectomized dogs*

Abstract

The influence of sympathetic tone on the action of disopyramide was assessed by studying the electrophysiological changes which followed the intravenous injection of 4 mg/kg of disopyramide into anesthetized dogs chemically sympathectomized with 6-hydroxydopamine. Atrial and ventricular pacing, together with recordings of the His bundle electrogram allowed assessment of the sinus node function, measurement of the atrioventricular and ventricular conduction intervals, and the detection of the supernormal conduction phenomenon. The duration of atrial (MAPa) and ventricular (MAPv) monophasic action potentials recorded by suction electrodes and their relation to the effective (ERP) and functional (FRP) refractory periods were measured in the atria and ventricles; the ERP/MAP ratio was also measured. The results show that the lengthening of the basic sinus cycle length and of the atrial and ventricular MAP, ERP and FRP observed after disopyramide is not only masked by the well-known anticholinergic effect of this agent but also by the degree of sympathetic tone. The integrity of the sympathetic nervous system is a requirement for the presence of effective antiarrhythmic effects of disopyramide in the ventricle.
