The European Society of Cardiology and the Publisher regret that in the task force report, two incorrect figures have been published. Fig. 2(page 1816) should be replaced by the figure below.

Furthermore, on page 1810, in the second column, line 25 should read ‘In these guidelines, the level of evidence and the strength of recommendation are summarized in table 5’.
